Extension Applications and Reissuance for Expiring Student ID cards, ID Cards, and Library Cards

For those whose student ID card, ID card, or library card expires at the end of the academic year, library privileges remain valid as follows:

      Students: until the day of the graduation ceremony
      Faculty/staff: until the end of March

If it has already been decided that you will remain enrolled or employed at the university after April and you wish to continue using the library, you are required to apply for an extension of your library privileges or for reissuance of your card.
Please follow the procedures outlined below to apply.

Students whose student ID card is about to expire

If you fall under any of the following categories, your library privileges will be extended.

①Students advancing to graduate school at Ochanomizu University 　→　Until the day of the entrance ceremony
②Students who will remain enrolled due to a leave of absence or similar circumstances 　→　Until the date of issuance of the reissuance schedule
③ Students who wish to continue using the library until the end of the month following the graduation ceremony 　→　Until the end of March

※Students in category ② must separately renew their student ID cards at the Student and Career Support Division on the 2nd floor of the Student Service Building

Application Method

Please apply using the "[For Students]Temporary Library Access Extension Application Form".(Microsoft Forms, Japanese only)
 

・The extension will be processed on or after the next business day.
・Only applicants for whom the library has additional matters to communicate will be contacted via the email address they provided.
・If you receive a new student ID card, please start using the new one  immediately.

Faculty/Staff whose university-issued ID cards have expired

Faculty/Staff who will remain enrolled at Ochanomizu University next year will have their library privileges extended for approximately one and a half months.

Application Method

Please apply using the "[For Faculty and Staff]Temporary Library Access Extension Application Form".(Microsoft Forms, Japanese only)
 
・The extension will be processed on or after the next business day.
・ Only applicants for whom the library has additional matters to communicate will be contacted via the email address they provided.
・Complete the renewal procedure at the General Affairs, General Division (※) to reset your library access period to match your ID card's expiration date.
 
(※) You need to renew your ID card.

Please fill out the “身分証明書交付申請書” available on desknet's and submit it to mibunsyo@cc.ocha.ac.jp (General Affairs, General Division).

Applications submitted to the General Affairs, General Division by the end of March are expected to be processed by late April to May.

Library cards valid until the end of the fiscal year

If you visit the library after April, we will issue you a new library card.
The card you currently hold cannot be extended.
Please return any materials currently on loan by their due date.

Application Method

Please complete the designated fields of the pink “Library User Card Application Form” on the writing desk next to the Circulation Desk  and submit it there.

Weekdays 9:00 a.m. to 12:00 p.m. / 1:00 p.m. to 5:00 p.m. Immediately
Weekdays 12:00 p.m. to 1:00 p.m. After 1:30 p.m. on the same day
After 5:00 p.m. on weekdays, and weekends After 10:00 a.m. on the next business day
